Safety From The Silent Depths
As the SUBSAFE Quality Assurance and Safety Officer on our nuclear submarine, I am intimately familiar with the story of the USS Thresher. On April 10, 1963, the US Navy lost the Thresher and all 129 crew members on board due to one small faulty pipe. But when you really examine the Thresher's story, it becomes clear that the loss didn't result from the pipe alone. In fact, many of the decisions and circumstances that led to losing the Thresher are the same as what business, organization and service leaders are faced with today - all of it impacts safety.
